| When you book, you will receive an online form to complete providing us with information on your medical, physical and lifestyle history. We also need a 3-day food diary and clarity on your most important concern. By collecting information prior to the session, it allows us to complete a pre-assessment without wasting time on information gathering during consultation. On receiving your information, you will receive a Calendly consultation link to arrange an appointment. |
